Cronos Report: Towards a Paradigm Shift in Chronic Disease Care

Chronic care represents one of the biggest challenges for the sustainability of the National Health Service (NHS). For around five years now we have been discussing the need for a paradigm shift, from a care model focused on treating severe cases to a model that puts special emphasis on multi-pathological patients. The number of this type of patients is increasing for two reasons: our society is ageing, and scientific developments make it possible that more and more pathologies that used to have a fatal outcome now become chronic and people can live with them with an optimal quality of life.
